Palliative Care Advanced Practice Provider - 0.8 FTE

Job Details:
• Collaborate with an interdisciplinary team to care for patients with serious illness • Focus on reducing symptoms and stress of illness to improve quality of life for patients and caregivers • Provide patient-centered care addressing physical, mental, emotional, and existential effects of suffering • Ensure recommended treatments align with patient goals • Work with a team of 4 physicians, 4 nurse practitioners, and dedicated social worker and chaplain • Primarily provide inpatient palliative care with possibility of outpatient work in the future • See approximately 5-7 patients per day in a consultant role • Work rotating day shifts, primarily Monday-Friday with possibility of weekend coverage in future • Must have ability to work independently • Minimum of 1-year post-degree clinical CNS/APRN/PA experience • Experience in Palliative Medicine strongly preferred • For NP: Requires Master's degree in nursing, current RN and NP licensure in Minnesota • For PA: Requires Bachelor's degree in health/science-related field, current PA licensure in Minnesota • Must maintain current CPR certification and national certification in applicable area • Must complete credentialing process prior to practice • Position requires high level of customer service skills
