Outpatient-only assignment in Lafayette with a light patient load and flexible weekday schedule.

Practice Setting

• Team-based clinic with MD, 2 NPs, RNs, CMAs, and admin staff
• Outpatient family medicine for all ages
• EMR: Athena

Schedule

• Mondays, Tuesdays, and Thursdays
• 7:00 AM-6:00 PM with 1-hour lunch
• 30 hours per week
• Initial 60-day contract with extension possible

Cases and Procedures

• General primary care
• DOT and employment physicals
• No procedures required

Requirements

• Certified Family Nurse Practitioner
• Active Indiana license and DEA
• DOT certification preferred but not required
