Job Title

IHA Per Diem Nurse Practitioner / Physician Assistant

Location

Binghamton, NY (Travel required within a 50-mile radius)

Temp-to-perm, 40 hours/week

Summary

We are seeking a Per Diem Nurse Practitioner or Physician Assistant to perform in-home, no-cost annual preventive health risk assessments for Medicare, Medicaid, and ACA members. Assessments are conducted in members’ personal living spaces and follow health plan expectations.

Key Updates

  • Candidates must have experience in primary care (PCP) or in-home health—ideally both.
  • Providers must be credentialed with Excellus, NYS Medicare, and Medicaid to avoid delays.
  • Expected volume: 25–30 visits/month.

Responsibilities

  • Conduct 60–75 minute in-home health assessments
  • Provide a minimum of 30 appointment slots/month
  • Submit chart notes within 24 hours of each visit
  • Respond to charting queries within 48 hours
  • Maintain high-quality documentation and member satisfaction
  • Attend training sessions and team meetings
  • Communicate effectively with clinical leadership
  • Lift up to 20 lbs and bend/kneel as needed during assessments

Schedule

Flexible hours (days/evenings)

Compensation

  • Market rate per completed assessment
  • Mileage reimbursed after first 30 miles/day at current IRS rate

Qualifications

Education & Experience

  • Master’s or Doctorate from an accredited NP program (FNP, AGNP, ANP) or PA program
  • Home health experience within the last 5 years (required)
  • 1+ year of in-home assessment experience (highly preferred)

Certifications & Licensure

  • Active RN and NP or PA license in practicing state
  • Board certification (ANCC, AANP, or NCCPA)
  • Current Medicaid ID and Medicare PTAN
  • BLS certification
  • Valid driver’s license and proof of auto insurance

Competencies

  • Proficiency with electronic medical records
  • Knowledge of CMS and ACA risk adjustment models
  • Strong verbal and written communication skills
  • Ability to make decisions within policy guidelines
  • Adaptability and collaborative mindset
  • Commitment to HIPAA compliance and confidentiality
