Job Title: Advanced Practice Clinician – Nurse Practitioner / Physician Assistant

Location: Skagit, Whatcom, and Northern Snohomish Counties, WA

Duration: 4–6 months

Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Travel: Local travel during shift between multiple facilities

Position Summary:

Seeking a dedicated Advanced Practice Clinician (APC) to provide integrated, longitudinal care to high-risk members in senior living settings. This role is part of an interdisciplinary team focused on improving outcomes and quality of life for patients with complex medical needs.

Primary Responsibilities:

• Conduct comprehensive clinical assessments
• Manage acute and chronic medical and behavioral conditions
• Ensure accurate and complete documentation, including ICD-10 coding
• Collaborate with interdisciplinary care teams
• Lead advanced illness and care planning conversations
• Provide education and counseling to patients and caregivers

Required Qualifications:

• Active and unrestricted license in the state of residence (or ability to obtain by start date)
• NP: Master of Science in Nursing or Doctor of Nursing Practice from an accredited program
• PA: Degree from an accredited Physician Assistant program
• Board certification:
• NP: AANP or ANCC (FNP, ANP, GNP, or AGACNP)
• PA: NCCPA
• Active DEA license/prescriptive authority
• Ability to meet physical job requirements (e.g., lifting 30 lbs, navigating stairs)
• Reliable transportation
• Ability to obtain collaborative practice agreement (if required by state)
• Fully vaccinated for COVID-19 (per federal, state, and client requirements)

Preferred Qualifications:

• 1+ year of clinical experience (preferably in long-term care)
• Experience with patients with complex behavioral, social, or functional needs
• Knowledge of geriatrics, chronic illness, and end-of-life care
• Proficiency in electronic medical records and documentation
