Position Description

Job Title: Nurse Practitioner – Community Rounding | Wound Certification Sponsored

Company: Twin Cities Physicians

Compensation: $105,000 to $120,000 annually

Benefits: Full Benefits Package (for full-time status)

Location: Greater Twin Cities Metro Area, MN

Schedule: Full-Time or Part-Time

About the Role:

Deliver mobile, patient-centered care to seniors
Focus on wound care and complex chronic condition management
Coordinate with RNs, families, and external specialists
Participate in interdisciplinary rounds and community huddles

Qualifications:

Minnesota-certified Nurse Practitioner (AGNP or CNP)
Wound care training provided if needed
Skilled at working independently and problem-solving
Compassion for aging adults
