Urgent Need: As soon as credentialed or by October 15 - end of year

Schedule: 8 hour days, 5 days a week (M-F or W-F example)

Hours of operation 8 AM - 8 PM (8 hour days only, this includes chart and travel time)

Scope of Practice/Responsibility:

Visit Type A. Level 2–3 Diagnostic Visits — close quality gaps for health plans:

1. A1c, bone density, KED, FIT test, diabetic retinal exam, etc.
2. Perform diagnostics, document results, follow-up steps

Visit Type B. Level 4 Comprehensive Home Annual — longer visit:

1. Full annual wellness + labs/diagnostics
2. Assess Social Determinants of Health; document needs/recommend resources
3. Care Guides convert signed charts into a 30-day community resource for the member

Visit Type C. Telehealth Annual Wellness — Limited
