Monmouth, Maine: We invite a skilled Nurse Practitioner or Physician Assistant to join a respected Federally Qualified Health Center serving a multicultural rural population of more than 10,000 people in central Maine. Founded in the 1970s, this nonprofit operates three clinics between Lewiston (Maine’s second-largest city) and Augusta (the state capital), each about an hour from Portland. Care is delivered in a nationally recognized Patient-Centered Medical Home model, with integrated behavioral health, care managers and patient assistance coordinators. This practice is well established, modern, and supported by experienced physicians, ensuring strong mentorship for a confident new graduate or early-career provider who shares a commitment to community-based care.

Candidates certified for all ages/family medicine or adult medicine are all welcome. This is a full-time position (4 days/week, 40 hours). You will enjoy a manageable patient panel providing comprehensive outpatient primary care to patients of all ages, with more seniors than pediatrics.
