Monmouth, Maine: We invite a skilled Nurse Practitioner or Physician Assistant to join a respected Federally Qualified Health Center serving a multicultural rural population of more than 10,000 people in central Maine. Founded in the 1970s, this nonprofit operates three clinics between Lewiston (Maine’s second-largest city) and Augusta (the state capital), each about an hour from Portland. Care is delivered in a nationally recognized Patient-Centered Medical Home model, with integrated behavioral health, care managers and patient assistance coordinators. This practice is well established, modern, and supported by experienced physicians, ensuring strong mentorship for a confident new graduate or early-career provider who shares a commitment to community-based care.
Candidates certified for all ages/family medicine or adult medicine are all welcome. This is a full-time position (4 days/week, 40 hours). You will enjoy a manageable patient panel providing comprehensive outpatient primary care to patients of all ages, with more seniors than pediatrics. Providers interested in addiction medicine will receive support to certify in this area. Become part of a collaborative physician/APP/LCSW team in a welcoming rural clinic environment.
