Our client is seeking a Primary Care Nurse Practitioner to join their growing team in Norfolk, VA. This is a newly created position based on recent/expected growth.

The role will require you to travel to local Dialysis Clinics, where you will see ~10 patients/day. There is occasional flexibility to work from home in the afternoons.

Schedule:

• Full Time: Monday-Friday, flexible start/end time, no nights or weekends
• On-call can be done telephonically (telephone triage only)

Compensation: $130,000 - $150,000/yr base salary, dependent on experience, plus a bonus structure that allows you to earn up to 60% of your base salary

Lead Nurse Practitioner Responsibilities:

• Perform comprehensive physical evaluations
• Develop individualized care plans and ensure appropriate follow up through the care continuum
• Work collaboratively with an interdisciplinary team consisting of Nephrologists, PCP’s, RN’s, Dieticians, Cardiologists, etc.
• Order labs and prescribe medications based on patient needs
• Provide coordinated care services between the patient, family, physicians and other care team members

Qualifications:

• Master’s Degree in Nursing
• Licensed Nurse Practitioner in the state of Virginia
• 4+ years of experience as a Nurse Practitioner within Primary Care or Internal Medicine
• Demonstrated expertise in chronic disease management
• Current DEA license
• Experience with dialysis or nephrology, preferred
