Position Description

Job Title: Nurse Practitioner – Remote/In-Field

Schedule: Monday–Friday, 8:00 AM – 5:00 PM (1-hour lunch)

On-Call: Rotation every 6–7 weeks with additional compensation

About the Role

The Nurse Practitioner will play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey. This position offers the opportunity to make a significant impact on patient outcomes and promote continuity of care. Patient visits will be completed in-home and/or in community-based settings such as assisted living facilities, skilled nursing facilities, group homes, or other delegated locations.

Must-Have Qualifications

  • Active Advanced Practice Provider (APP) License
  • DEA License
  • Minimum 1 year of prior experience as an APP

Preferred Qualifications

  • Medicare and Medicaid ID
  • Gerontology Certification/Specialty

Essential Functions

  1. In-Home Provider Visits: Conduct a minimum of 25 comprehensive in-home assessments per week, including medical history reviews, physical exams, and medication management, within assigned territory.
  2. Patient Assessment: Perform thorough evaluations of patients’ health status, including histories, physicals, and risk assessments.
  3. Diagnosis & Treatment: Develop accurate diagnoses and individualized treatment plans based on evidence-based practices.
  4. Chronic Disease Management: Monitor and manage chronic conditions (e.g., diabetes, hypertension, cardiovascular disease) while providing education to improve control and outcomes.
  5. Medication Management: Prescribe and adjust medications as appropriate while educating patients on safe use and side effects.
  6. Preventive Care: Promote preventive healthcare measures, such as screenings, immunizations, and lifestyle modifications.
  7. Patient Education: Provide education on disease management, self-care, and healthy living to empower patient involvement.
  8. Collaborative Care: Work closely with physicians, nurses, and other professionals to ensure coordinated and seamless care.
  9. Primary Care Role: Serve as the primary care provider of record for assigned patients.
  10. Documentation: Accurately complete EMR documentation within 48 hours of visits.
  11. Bedside Manner: Foster trust and rapport through empathy and patient-centered communication.
  12. Follow-Up Care: Conduct ongoing follow-up visits to monitor treatment effectiveness and adjust as needed.
  13. Quality Assurance: Participate in quality initiatives and adhere to care standards and regulations.
  14. After-Hours On-Call: Provide longitudinal care and IHA visits during periodic after-hours call rotations.
  15. Travel: Maintain reliable transportation for patient visits within assigned territories.
  16. Compliance: Adhere to all policies, procedures, codes of conduct, and compliance standards.
  17. Confidentiality: Comply with federal, state, and local privacy/security regulations.
  18. Other Duties: Perform additional responsibilities as assigned based on business needs.
