Nurse Practitioner – Longitudinal Care
Location: Glendale & Peoria, AZ
Schedule: Monday–Friday, 9:00 AM – 5:00 PM (40 hours/week)
Coverage Area: Within a 50-mile radius; may extend based on patient census
Position Summary
This Nurse Practitioner (NP) role focuses on delivering comprehensive, longitudinal care to patients in their homes or community-based settings such as assisted living facilities, skilled nursing facilities, or group homes. The NP will serve as the primary care provider, ensuring continuity of care and improved patient outcomes.

Position Summary
This Nurse Practitioner (NP) role focuses on delivering comprehensive, longitudinal care to patients in their homes or community-based settings such as assisted living facilities, skilled nursing facilities, or group homes. The NP will serve as the primary care provider, ensuring continuity of care and improved patient outcomes.
Key Responsibilities
- Conduct a minimum of 25 in-home visits per week
- Perform thorough patient assessments, including medical history, physical exams, and risk evaluations
- Diagnose and develop individualized treatment plans
- Manage chronic conditions (e.g., diabetes, hypertension, cardiovascular disease)
- Prescribe and manage medications
- Promote preventive care and healthy lifestyle practices
- Educate patients on disease management and self-care
- Collaborate with interdisciplinary healthcare teams
- Maintain accurate EMR documentation within 48 hours of visits
- Provide empathetic, patient-centered care
- Conduct follow-up visits and adjust care plans as needed
- Participate in quality improvement initiatives
- Be available for after-hours on-call duties as needed
- Travel reliably to patient appointments
- Active, unrestricted NP license in Arizona
- Certified as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner
- DEA license and current BLS certification
- Minimum of 2 years in primary or longitudinal care preferred
- Strong clinical assessment and diagnostic skills
- Proficiency in EMR systems
- Reliable transportation and valid driver’s license (minimum 3 years)
- Ability to pass comprehensive background and drug screening
- HEDIS/STAR measures
- Home-based care
- HCC documentation
- Ability to lift/carry up to 20 lbs
- Comfortable with standing, bending, kneeling, and navigating various home environments
- Mental agility for data interpretation, decision-making, and planning
- Exposure to diverse home conditions and environments
- Self-motivated and independent
- Strong communication and documentation skills
- Empathetic and respectful patient interactions
- HIPAA compliance and confidentiality
- Adaptability and problem-solving
- Collaborative and proactive approach to care
