Overview

We are seeking Field Nurse Practitioners (NPs) to join our Care Connections team. This role focuses on providing preventive and primary care services directly in the home, community, and nursing facility settings. You’ll conduct initial comprehensive assessments to identify gaps in care and promote wellness — all while working in environments where patients feel most comfortable.

• ~45 minutes per visit
• 6–9 visits/day (depending on shift)
• Chart in the field using Epic EMR
• Travel within a 60-mile radius
• Initial assessments only – no ongoing patient care
• 5-week mandatory training (paid)

Key Responsibilities

• Perform comprehensive medical evaluations, including history and physical exams for preventive and diagnostic purposes.
• Address both acute and chronic care needs, and determine appropriate medical urgency.
• Establish diagnoses, order labs/imaging as needed, and coordinate specialty consultations.
• Create and implement individualized medical care plans.
• Conduct face-to-face visits in patient homes, community clinics, or skilled nursing facilities.
• Participate in community “Pop-Up Clinics” and initiatives addressing care gaps.
• Provide post-discharge coordination to reduce readmission and ER utilization.
• Collaborate with regional NP teams and participate in team meetings.
• Drive up to 120 miles per day within assigned territory (occasional overnight travel may be required).
• Maintain timely and accurate documentation in Epic.

Qualifications

Must-Have Skills / Requirements

• Licensed Adult or Family Nurse Practitioner
• Comfortable with 100% fieldwork and home visits
• Must reside within 20 minutes of assigned territory
• Clear background and ability to complete 5-week onboarding/training
• Valid driver’s license and reliable transportation

Schedule Options

Option 1 – 4x10 (Mon–Fri):

• 8:00 AM – 7:00 PM
• Must include 1 Saturday per month
• 8–9 visits per day

Option 2 – 5x8 (Mon–Sat):

• 3 days: 8:00 AM – 5:00 PM
• 2 days: 10:00 AM – 7:00 PM
• Must maintain a consistent weekly schedule
• 6–7 visits per day
