Job Summary
We are looking for a dedicated and compassionate Primary Care Provider to join our team in a long-term care setting.
This full-time position is based in Westchester, NY, and requires an onsite presence.
The role involves providing comprehensive care to patients, working closely with a team of healthcare professionals, and requires a willingness to relocate to New York.
Requirements
Provide comprehensive healthcare services and coordinate patient care
Conduct thorough patient assessments, including medical history reviews and physical examinations
Order and interpret diagnostic tests
Develop accurate diagnoses and treatment plans
Prescribe medication and administer treatment
Collaborate with the interdisciplinary team for seamless coordination of care
Manage chronic conditions and monitor progress
Qualifications
Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
State-specific license in good standing in New York
Minimum 6 months of experience in long-term care or a skilled nursing facility
Strong communication, clinical, and critical thinking skills
Passion for elder care and person-centered healthcare
Ability to relocate to New York
