Our client is currently seeking a Nurse Practitioner in Orange County, FL to complete home visits.

Job Summary

This is a Monday to Friday, 8 am to 5 pm position completing approximately 5-7 visits per day. The NP will work alongside a Registered Nurse and collaborate with the patient's primary care provider to identify and address red flags and issues that can lead to hospitalization or an ER visit. The NP will see patients in their home based on their needs, ranging from twice a month to once every three months. Ultimately, the goal is to keep the patient healthy at home, while working closely with their PCP and specialists.
