Position Description

Position Title: IHA Per Diem Nurse Practitioner / Physician Assistant

Reports To: Manager of Care Delivery

FLSA Status: Exempt

Location Coverage: Marion County, FL — including Ocala, Belleview, Dunnellon, Silver Springs, and Summerfield

Travel Requirement: Up to a 50-minute radius from home base

Monthly Visit Expectation: 25–30 in-home assessments

Schedule: Flexible days and evenings, with 60–75 minute appointment blocks

Compensation: Competitive per-assessment rate; mileage reimbursed after the first 30 miles/day at the current IRS rate

Position Summary

This role involves conducting in-home, no-cost annual preventive health risk assessments for Medicare, Medicaid, and ACA members. Assessments are performed in the member’s personal living space and are designed to support care planning and improve health outcomes.

Preferred Experience

Candidates with Primary Care and/or Home Health experience are strongly preferred. Ideally, applicants will have both.

Essential Responsibilities

  • Complete high-quality health risk assessments in members’ homes
  • Submit chart notes within 24 hours of each visit
  • Respond to charting queries within 48 hours
  • Maintain clear communication with clinical leadership
  • Attend required training sessions and team meetings
  • Safely enter and work in members’ homes
  • Lift up to 20 lbs and kneel/bend as needed during assessments

Required Qualifications

  • Master’s or Doctorate from an accredited NP program (FNP, AGNP, ANP) or PA program
  • Active RN and NP or PA licensure in the state of practice
  • Board certification (ANCC, AANP, or NCCPA)
  • Current BLS certification
  • Valid Medicaid ID and Medicare PTAN
  • Valid driver’s license and proof of auto insurance
  • Home health experience within the last 5 years
  • At least 1 year of in-home assessment experience (highly preferred)

Skills & Competencies

  • Proficiency with electronic medical records (EMR)
  • Understanding of CMS and ACA risk adjustment models
  • Strong documentation and communication skills
  • Ability to maintain HIPAA compliance and confidentiality
  • Critical thinking and decision-making skills
  • Adaptability and collaborative mindset
  • Ability to proactively resolve member and provider issues
