Compensation: $110,000-$120,000 per year

• Quarterly incentive bonuses (3–5% of salary, based on department productivity)

Position Overview

We are seeking a dedicated Advanced Practice Provider (APP) to join our team. While a Nurse Practitioner (NP) is preferred, we are open to Physician Assistants (PAs) with a strong nursing or clinical background. This role is ideal for an experienced provider who thrives in a primary care or integrated health setting and has a passion for serving diverse patient populations, including those commonly seen in Federally Qualified Health Centers (FQHCs).

This position offers the opportunity to build and manage your own patient panel, while also having the support of a collaborating physician. Providers typically see 15–16 patients per day with 20-minute appointment times—allowing for more meaningful patient interactions compared to the standard 15 minutes.

New graduates will not be considered for this position.

Key Responsibilities

• Examine, diagnose, and treat patients of all ages for a wide range of conditions.
• Provide well-child, well-woman, and well-man preventive health exams.
• Order, interpret, and follow up on screening, diagnostic, and lab results.
• Develop individualized care plans and prescribe medications and treatments as appropriate.
• Deliver age-appropriate, culturally competent, and patient-centered care.
• Promote health education, disease prevention, and patient empowerment.
• Refer patients to specialty care as appropriate and coordinate care with specialists.
• Maintain accurate and timely patient documentation.
• Participate in call rotation as needed.
• Provide instruction or mentorship to medical or nursing students when appropriate.
• Operate within scope of practice under Ohio state law and agreed-upon collaborative agreements.
• May be asked to provide coverage at multiple clinic locations as needed.

Qualifications

• Current and unrestricted Nurse Practitioner or Physician Assistant license in Ohio.
• National certification as NP or PA.
• Minimum of 2+ years’ experience in primary care, integrated health, or FQHC setting.
• Strong clinical judgment and ability to manage an independent patient panel.
• Excellent communication, collaboration, and patient relationship-building skills.
• Supervisory or administrative experience a plus.
