Penn Medicine Advanced Home Health is searching for an experienced, creative thinking CRNP to join our team. This practice serves a unique population of geriatric patients, requiring both inpatient and outpatient skills as patients in the program are seen in the home as well as in the hospital setting. Most of the CRNP’s time will be spent in the home setting. The ideal candidate will be comfortable with their nursing skills, as most in-home care is done by the CRNP. This role offers a great deal of autonomy with minimal call and a flexible schedule.

Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
- Consults with patients to determine the appropriate course of treatment.
- Recommends and orders tests to determine extent of illness or help diagnose condition.
- Monitors lab results, medication adherence and medication toxicities.
- Prescribe or administer treatment, therapy, medication, vaccinations, and other medical care to treat or prevent illness, disease, or injury.
- Explain and discuss procedures, test results, or prescribed treatment with the patient.
- Coordinate work with nurses, social workers, rehabilitation therapists, pharmacists, psychologists, and other health care providers.
- Refers patients to medical specialists when necessary.
- Current Licensure as a Nurse Practitioner (CRNP). Only Adult-Gero and Acute Care certifications will be considered.
- Valid, unrestricted Federal Drug Enforcement Agency (DEA) number
- Participation as a provider in good standing in the Medicare, Medicaid, and Tricare programs and such other third-party-payment programs, including managed care programs in white LGMG participates.
