Transitional Care Nurse Practitioner

Our client is looking for a compassionate Nurse Practitioner to lead transitional care for high-risk patients, helping reduce hospital readmissions and improve quality of life for those with chronic conditions like CHF and COPD. You will guide patients from post-discharge to stability, providing care at home and in clinic settings.

What You’ll Do

• Deliver comprehensive, evidence-based care to high-risk patients
• Coordinate timely follow-ups and connect patients with social and behavioral health services
• Monitor and document progress to ensure sustained recovery
• Collaborate with cross-functional teams to improve outcomes and patient satisfaction

What We’re Looking For

• Licensed Nurse Practitioner (CA)
• 3–4 years of clinical experience, ideally with COPD/CHF or complex patient populations
• Home-care experience preferred
• Strong communication and documentation skills (Spanish a plus)
