Role Description

This is a full-time on-site role for a Nurse Practitioner located in Los Angeles, CA. The Nurse Practitioner will be responsible for providing comprehensive wound care services including assessment, treatment, and management of acute and chronic wounds. The role involves coordinating with facility staff and primary care providers to create and implement care plans, monitoring patient progress, and advocating for patient health and well-being. The Nurse Practitioner will work to ensure the highest level of patient care in a compassionate and collaborative environment.

Qualifications
  • Advanced clinical skills in wound care, assessment, and treatment of acute and chronic wounds
  • Experience with patient management in home health agencies, skilled nursing facilities, or assisted living communities
  • Strong coordination abilities with facility staff and primary care providers
  • Excellent communication and interpersonal skills
  • Compassionate approach towards patient care and well-being
  • Ability to work independently and collaboratively in a clinical setting
  • Current certification as a Nurse Practitioner and relevant state licensure
  • Experience in post-acute and long-term care settings is a plus
