About the Role
We are seeking an experienced and compassionate Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) to deliver Home-Based Primary Care services to patients in their residences or Assisted Living communities. In this role, you will manage your own patient panel, diagnosing, treating, and managing both acute and chronic medical conditions under physician oversight. This position is ideal for a clinician who values autonomy, continuity of care, and meaningful relationships with patients in their home environment.
Key Responsibilities
- Perform comprehensive in-home assessments and collaborate with the primary care team to optimize chronic disease management.
- Review current symptoms, identify diagnoses, and manage treatment plans for acute and chronic conditions.
- Complete physical exams, medication reviews, vital signs checks, lab draws, wound care, and depression screenings.
- Communicate clinical findings and care recommendations to the Primary Care Physician to ensure continuity and quality of care.
- Support accuracy in risk capture and quality metric performance while achieving high patient satisfaction and improved outcomes.
- Provide education to patients and caregivers regarding disease processes, medications, and treatment compliance.
- Use critical thinking to address patient needs, coordinate resources, and ensure appropriate follow-up for stability and safety.
- Comply with all HIPAA regulations and protect patient health information.
- Travel daily to provide care in patients’ homes or community settings as needed.
Qualifications
- Licensed Family Nurse Practitioner (FNP), APRN, or Physician Assistant (PA) in the state of Indiana.
- Minimum of 2+ years of clinical experience in primary care or home-based care.
- National certification from ANCC, AANP, or NCCPA.
- Working knowledge of chronic care management and current pharmacologic therapies.
- Current DEA license (or ability to obtain post-hire).
- Experience in Family Medicine, Internal Medicine, Gerontology, or Palliative Care preferred.
- Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification required.
