We are seeking a compassionate, dedicated, and team-oriented Nurse Practitioner OR Physician Assistant to serve as a Primary Care Provider (PCP) at our PACE location. The Advanced Practice Provider will deliver participant-centered care within an interdisciplinary team, directly contributing to the health outcomes and quality of life of our participants. This role requires a combination of direct patient care, team support, and clinical administrative responsibilities. Schedule: Monday-Friday 8am-5pm.

Pay range: $135,400 - $211,200 annually, depending on experience.
Compensation Disclosure: The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.

Responsibilities
Direct Patient Care:
- Provide comprehensive primary care to PACE participants, expertly managing acute and chronic health conditions.
- Perform initial and ongoing comprehensive physical examinations and thorough health assessments.
- Order, administer, and accurately interpret diagnostic tests such as laboratory tests, electrocardiograms, and imaging studies.
- Conduct therapeutic procedures, including injections, immunizations, wound care, and managing infections.
- Offer preventive care services, medication management, and coordinate necessary specialty care services.
- Conduct home visits to evaluate participant safety, functionality, and overall well-being, ensuring comfort and dignity in their living environment.
- Educate and counsel participants and families about prescribed therapies, health maintenance strategies, and lifestyle modifications.
- Identify and address psychosocial, emotional, and environmental factors affecting participant health.
- Collaborate extensively with an interdisciplinary team including physicians, nurses, therapists, social workers, dietitians, behavioral health specialists, home care coordinators, and support staff.
- Actively participate in weekly interdisciplinary team meetings to review, revise, and enhance care plans, ensuring the coordinated delivery of comprehensive care.
- Foster strong professional relationships across all team levels, promoting clear communication, coordinated actions, and positive team dynamics.
- Share responsibility for achieving overall participant health outcomes, consistently supporting and contributing to team goals and objectives.
- Provide guidance and mentorship to other clinical team members, supporting continuous professional development within the team.
- Develop, implement, and continuously update individualized care management plans, maintaining precise and comprehensive documentation in the electronic health record (EHR).
- Ensure continuity and consistency of care through accurate and timely documentation, and communicate updates effectively with the interdisciplinary team.
- Adhere to established Neighborhood Healthcare practice protocols and perform duties under the supervision of the Regional Medical Director and collaborating physicians.
- Stay current on organizational goals, strategic activities, and any changes in healthcare policies and practices affecting PACE services.
- Contribute actively to quality improvement initiatives aimed at enhancing participant outcomes and optimizing clinical workflows.
Education/Experience
- Master’s degree from an accredited physician assistant or nurse practitioner program required
- Valid CA Physician Assistant or Nurse Practitioner license required
- Valid DEA License required
- Current Basic Life Support (BLS) certification through an American Heart Association (AHA) approved source required upon hire and must be maintained as a condition of employment.
- NPs: Board Certification (ANCC/AANPCB) or board eligible with certification within 12 months of hire, required.
- Preferred experience includes geriatrics, chronic disease management, palliative care, home-based care, or skilled nursing environments.
- Exceptional interpersonal and teamwork skills, demonstrating passion for collaborative, participant-centered care.
- Superior verbal and written communication skills, with the ability to communicate effectively with diverse populations.
- Proven ability to efficiently manage multiple tasks and priorities simultaneously.
- Strong organizational skills, attention to detail, and meticulousness in clinical documentation.
- Commitment to maintaining confidentiality, professional integrity, and ethical behavior in all clinical interactions.
