Role Summary
We are seeking a Full-Time Nurse Practitioner (NP) or Physician Assistant (PA) to deliver high-quality, patient-centered care in skilled nursing and post-acute settings. This role combines clinical excellence with innovative technology, including Remote Patient Monitoring (RPM), to ensure continuous, comprehensive care for patients.
Responsibilities
We are seeking a Full-Time Nurse Practitioner (NP) or Physician Assistant (PA) to deliver high-quality, patient-centered care in skilled nursing and post-acute settings. This role combines clinical excellence with innovative technology, including Remote Patient Monitoring (RPM), to ensure continuous, comprehensive care for patients.
Responsibilities
- Provide daily clinical care for residents in assigned skilled nursing facilities.
- Conduct histories, physical exams, diagnoses, and develop treatment plans.
- Prescribe medications, order diagnostics, and monitor patient progress.
- Educate patients and families on health maintenance and disease prevention.
- Document all encounters accurately and promptly using facility EHR systems (PointClickCare, MatrixCare, American HealthTech).
- Collaborate with supervising physicians and facility staff to ensure quality outcomes.
- Participate in chart reviews and quality audits for ongoing improvement.
- Maintain Medicaid enrollment and compliance with state regulations.
- Be available for phone or secure messaging consultations with nursing staff.
- Ensure reliable transportation and punctual attendance for scheduled shifts.
- Active Nurse Practitioner or Physician Assistant license in the state of employment.
- Medicaid enrollment in the state of practice.
- Experience in skilled nursing or post-acute care settings.
- Proficiency in clinical documentation and EHR systems (training provided).
- Reliable transportation for travel between facilities.
- Prior experience with PointClickCare, MatrixCare, or American HealthTech.
- Strong communication and collaboration skills.
- Ability to work independently and manage time effectively.
- Schedule: Monday–Friday, 7:00 AM–1:00 PM (flexible start times), no weekends or holidays.
