Role Summary

We are seeking a Full-Time Nurse Practitioner (NP) or Physician Assistant (PA) to deliver high-quality, patient-centered care in skilled nursing and post-acute settings. This role combines clinical excellence with innovative technology, including Remote Patient Monitoring (RPM), to ensure continuous, comprehensive care for patients.

Responsibilities

• Provide daily clinical care for residents in assigned skilled nursing facilities.
• Conduct histories, physical exams, diagnoses, and develop treatment plans.
• Prescribe medications, order diagnostics, and monitor patient progress.
• Educate patients and families on health maintenance and disease prevention.
• Document all encounters accurately and promptly using facility EHR systems (PointClickCare, MatrixCare, American HealthTech).
• Collaborate with supervising physicians and facility staff to ensure quality outcomes.
• Participate in chart reviews and quality audits for ongoing improvement.
• Maintain Medicaid enrollment and compliance with state regulations.
• Be available for phone or secure messaging consultations with nursing staff.
• Ensure reliable transportation and punctual attendance for scheduled shifts.
  
  

Minimum Qualifications

• Active Nurse Practitioner or Physician Assistant license in the state of employment.
• Medicaid enrollment in the state of practice.
• Experience in skilled nursing or post-acute care settings.
• Proficiency in clinical documentation and EHR systems (training provided).
• Reliable transportation for travel between facilities.
  
  

Preferred Qualifications

• Prior experience with PointClickCare, MatrixCare, or American HealthTech.
• Strong communication and collaboration skills.
• Ability to work independently and manage time effectively.
