Position Description

Join a mission-driven healthcare team as a Field Nurse Practitioner / Physician Assistant in Casa Grande, AZ, where you’ll deliver high-quality, in-home care to patients with chronic kidney disease. Enjoy the autonomy of field-based work, a flexible weekday schedule with no on-call, and the support of an interdisciplinary team — all while earning competitive pay, a $10K sign-on bonus, and up to $10K in relocation assistance.

Pay Details: $10,000 Sign-On Bonus!

  • 10% Annual Bonus
  • Up to $10k Relocation Bonus if needed
  • $140,000 - $150,000 a year (based on experience)

Shift: Monday – Friday | 8a-5p | No on call!

Daily Responsibilities for the Field Nurse Practitioner / Physician Assistant:

  • Provide in-home and virtual primary care for CKD patients, working with a care team
  • Oversee assessments, planning, and coordination; flexible schedule; geriatrics/nephrology experience preferred

Required Qualifications for the Field Nurse Practitioner / Physician Assistant:

  • Active licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Arizona
  • Must be comfortable interacting with patients and care teams both telephonically and in-person
