  • Position: Locum Tenens / PRN Nurse Practitioner – Home Health Risk Assessments
  • Location: Richmond, VA (county-based coverage; travel required up to 60-mile radius)
  • Shifts per Month: 4 shift minimum
  • Shift Duration: 8-hour field days (includes travel time)
  • Schedule: Flexible start times (7:00 AM or 8:00 AM); minimum 4 full days/month availability (Mon–Fri preferred); weekend shifts available
  • Estimated Pay: $480 - $533/week (per assessment pay model; $60/day travel stipend)
  • Assignment Length: Ongoing / PRN
  • Travel: Daily travel to patient homes (over 50% travel; up to 120 miles/day)

Nurse Practitioner Qualifications

  • Active, unrestricted Nurse Practitioner license in Virginia (ANCC, AANP, or AACN Board Certification required)
  • Accepted NP specialties: AGNP (Adult Geriatric Primary Care), FNP (Family Nurse Practitioner), ENP (Emergency Nurse Practitioner), ACNP (Acute Care Nurse Practitioner)
  • Minimum 1 year of Nurse Practitioner experience in a clinical or home health setting (independent practice required; new graduates not accepted)
  • Physician Assistants (PAs) with active Virginia license, board certification, and 1 year of PA experience also considered
  • Comfortable traveling independently to patient homes in rural, urban, and underserved areas
  • Strong commitment to patient care, adaptability, and flexibility in dynamic healthcare environments
  • Ability to provide own patient bag with basic medical tools
  • Professional attire or dark-colored scrubs (no lab coats needed)

Home Health Nurse Practitioner Responsibilities

  • Conduct annual Health Risk Assessments for geriatric patients in their homes
  • Perform physical exams, medication reviews, and assess social and environmental factors impacting patient health
  • Perform basic labs (e.g., A1C, fit tests; no blood draws required)
  • Document and submit assessments using a provided tablet (same-day submission required)
  • Travel daily to patient homes within assigned territory (up to 120 miles/day)
  • Adapt to changing routes, manage no-shows, cancellations, and logistical changes
  • Maintain high quality scores and compliance with documentation standards
  • Collaborate with the Clinical Services team for ongoing support and quality assurance
