Home Health Nurse Practitioner Job Details

• Healthcare Setting: Home Health – Geriatric Risk Assessments

• Duration: Ongoing, PRN healthcare opportunity
• Shifts per Week: Minimum 1 shift/week (at least 4 full days/month; half-days allowed after minimum)
• Shift Duration: 8-hour field days (includes travel time)
• Estimated Weekly Pay: $767 - $852 (per assessment payment model)
• Travel: Daily travel to patient homes (up to 120 miles/day depending on territory)

Nurse Practitioner & Physician Assistant Qualifications

• Active, unrestricted Nurse Practitioner (NP) license in Virginia (ANCC, AANP, or AACN Board Certification required)
• Accepted NP specialties: Adult Geriatric Nurse Practitioner (AGNP), Family Nurse Practitioner (FNP), Emergency Nurse Practitioner (ENP), Acute Care Nurse Practitioner (ACNP)
• Physician Assistants (PA) accepted in Virginia – must be board certified
• Minimum 1 year of independent NP or PA experience in clinical or home health settings (no new grads)
• Comfortable with solo travel to patient homes, including rural and underserved areas
• Strong adaptability to route changes, no-shows, and schedule variability
• Exceptional compassion, flexibility, and professionalism in healthcare delivery
• Valid driver’s license and reliable transportation required

Home Health Nurse Practitioner Responsibilities

• Perform annual in-home Health Risk Assessments for geriatric patients (1:1 visits, no follow-up care required)
• Conduct comprehensive physical exams, medication reviews, and assess social/environmental factors impacting patient health
• Complete basic labs (e.g., A1C, fit tests – no blood draws required)
• Document and submit assessments using a company-issued tablet (same-day submission required)
• Travel daily to assigned patient homes within a typical 60-mile radius of Rockwood, VA
• Maintain high quality scores and adhere to all clinical and documentation standards for home health jobs
• Communicate effectively with the clinical services team and operations manager
• Represent the organization with professionalism and empathy in every patient interaction
