Key Job Details – Nurse Practitioner Opportunity

• Location: Montgomery County, OH 
• Employment Type: Locum Tenens, PRN/Part-Time Nurse Practitioner
• Shifts per Week: 5 (minimum 4 full days/month; weekday coverage preferred, weekends a plus)
• Shift Duration: Part-Time, 8-hour field days (flexible start at 7:00 or 8:00 AM)
• Start Date: November 17th, 2025
• Assignment Duration: Ongoing, monthly scheduling

Nurse Practitioner Qualifications

• Active, unrestricted Nurse Practitioner (NP) license in Ohio
• Board certification: ANCC, AANP, or AACN
• At least 1 year of NP clinical experience (new graduates not eligible)
• Accepted specialties: Adult Geriatric Primary Care Nurse Practitioner (AGNP), Family Nurse Practitioner (FNP), Emergency Nurse Practitioner (ENP), Acute Care Nurse Practitioner (ACNP)
• Comfortable traveling independently to patient homes in rural, urban, and suburban areas
• Adaptable to route changes, schedule adjustments, and logistical unpredictability
• Passionate about patient care with a flexible, proactive mindset
• Minimum availability: 4 full days/month (half-days allowed after minimum met)
• Professional attire or dark-colored scrubs required (no lab coat needed)
• Must provide own patient bag with basic medical tools

Primary Responsibilities – In-Home Risk Assessment Nurse Practitioner

• Perform annual in-home health risk assessments for geriatric patients
• Conduct physical exams, medication reviews, and assess social/environmental factors
• Perform basic labs (e.g., A1C, fit tests; no blood draws required)
• Document and submit assessments using a provided tablet (same-day submission required)
• Travel daily to patient homes within a typical 60-mile radius
• Increase from 2–3 visits/day to 4–6 visits/day after training
• Maintain high quality scores and compliance with documentation standards
• Collaborate with operations and clinical services teams for scheduling and support
