Position Description

Job Title: Advanced Practice Clinician – Nurse Practitioner / Physician Assistant

Work Location:

Albany, NY (Albany and Rensselaer counties)

  • Travel between 2 facilities during shift
  • Reliable transportation required

Work Hours:

Full-time, 40 hours/week

  • Monday–Friday, 8:00 AM – 5:00 PM
  • No on-call, weekends, or holidays

Position Overview:

The Senior Community Care (SCC) program is a longitudinal, integrated care delivery model that coordinates clinical care for members in facility settings. The Advanced Practice Clinician (APC) provides care to high-risk health plan members as part of an interdisciplinary team. This role offers flexible scheduling and work-life balance, with no on-call requirements. Primary Responsibilities:

  • Conduct comprehensive assessments of members
  • Manage acute and chronic medical and behavioral conditions
  • Ensure accurate documentation, including ICD-10 coding
  • Collaborate with interdisciplinary care teams
  • Lead advanced illness and care planning conversations
  • Provide counseling and education to patients and caregivers

Required Qualifications:

  • Active, unrestricted NP or PA license in state of residence (or ability to obtain by start date)
  • Education:
  • NP: Master of Science in Nursing or Doctor of Nursing Practice
  • PA: Accredited Physician Assistant degree program
  • Active national board certification:
  • NP: AANP or ANCC (Family NP, Adult NP, Gerontology NP, Adult-Gerontology Acute Care NP)
  • PA: NCCPA certification
  • Current DEA licensure/prescriptive authority
  • Ability to meet physical requirements (lifting 30 lbs, navigating stairs, kneeling for assessments)
  • Reliable transportation
  • Ability to obtain collaborative practice agreement if required by state
  • Full COVID-19 vaccination (proof required prior to employment)

Preferred Qualifications:

  • 1+ year clinical experience (long-term care preferred)
  • Experience managing complex behavioral, social, and functional needs
  • Knowledge of geriatrics, chronic illness, and end-of-life care
  • Proficient in electronic medical records and documentation
  • Strong computer skills
