Position Description
Location: Lee County, FL (within a 50-minute driving radius from home address)
Schedule: PRN – Minimum of 30 visits per month
Position Overview
We are seeking a dedicated Per Diem Nurse Practitioner or Physician Assistant to perform in-home, no-cost annual preventive health risk assessments for members enrolled in Medicare, Medicaid, and ACA health plans. These assessments are conducted in the member’s personal residence and are designed to meet health plan quality and satisfaction standards.
Key Responsibilities
- Conduct comprehensive health risk assessments in members' homes.
- Provide a minimum of 30 appointment slots per month.
- Submit chart documentation within 24 hours of each visit.
- Respond to charting queries within 48 hours.
- Maintain clear and professional communication with clinical leadership.
- Attend required training sessions and team meetings.
- Ensure assessments meet quality and satisfaction benchmarks.
- Travel within a 50-minute radius of home address.
Compensation & Benefits
- Competitive market rate per completed assessment.
- Mileage reimbursement after the first 30 miles driven daily (at current IRS rate).
- Flexible scheduling (day and evening appointments available).
- Each appointment is scheduled for 60–75 minutes.
Required Qualifications
Education & Experience:
- Master’s or Doctorate degree from an accredited NP program (FNP, AGNP, ANP) or PA program.
- Minimum 1 year of in-home assessment experience preferred.
- Home health experience preferred.
Licensure & Certifications:
- Active RN and NP or PA license in the state of practice.
- Board certification (ANCC, AANP, or NCCPA).
- Current Medicaid ID and Medicare PTAN number.
- Valid BLS certification.
- Unencumbered state driver’s license.
- Proof of adequate auto insurance coverage.
Skills & Competencies
- Proficiency with electronic medical records and strong computer skills.
- Understanding of CMS and ACA risk adjustment models.
- Excellent verbal and written communication skills.
- Ability to make sound clinical decisions and follow established protocols.
- Strong organizational skills and attention to detail.
- Ability to maintain confidentiality and comply with HIPAA regulations.
- Physical ability to lift up to 20 lbs and kneel/bend as needed during assessments.
- Comfortable entering members’ homes for scheduled visits.
