Job Title: 
Advanced Practice Clinician - Nurse Practitioner / Physician Assistant
Location: Fayetteville, NC (Travel between 2-3 facilities)
Duration: Approx. 3 months (Potential for hire)
Schedule:
  • Monday-Friday, 8:00 AM - 5:00 PM
  • No on-call, weekends, or holidays
  • 40 hours per week
Program:

Senior Community Care (SCC) - ISNP

Overview:

The Senior Community Care (SCC) program is a longitudinal, integrated care delivery model that coordinates clinical care for members in facility settings. As an Advanced Practice Clinician (APC), you will provide care to high-risk health plan members as part of an interdisciplinary team. This role offers work-life balance with flexible scheduling and no on-call requirements.

Primary Responsibilities:
  • Conduct comprehensive assessments
  • Manage acute and chronic medical and behavioral conditions
  • Ensure accurate documentation, including ICD-10 coding
  • Collaborate with interdisciplinary care teams
  • Lead advanced illness and care planning conversations
  • Provide counseling and education to patients and caregivers
Required Qualifications:
  • Active, unrestricted NP or PA license in state of residence (or ability to obtain)
  • NP: Graduate of accredited MSN or DNP program
  • PA: Graduate of accredited PA program
  • Board certification:
    • NP: AANP or ANCC (Family, Adult, Gerontology, Adult-Gerontology Acute Care)
    • PA: NCCPA
  • Current DEA licensure/prescriptive authority
  • Ability to meet physical requirements (lifting 30 lbs, navigating stairs, performing assessments)
  • Reliable transportation
  • Ability to obtain collaborative practice agreement if required
  • Full COVID-19 vaccination
Preferred Qualifications:
  • 1+ year clinical experience (long-term care preferred)
  • Experience with complex behavioral, social, and functional needs
  • Knowledge of geriatrics, chronic illness, and end-of-life care
  • Proficient in EMR documentation
