LSSI is hiring a full-time Nurse Practitioner at Belmont Outpatient Community, Monday through Friday from 9:00am to 5:00pm. Responsible for providing evidence-based prescribing practices and interventions with an emphasis on shared decision-making, tailoring treatment to an individual client’s symptoms and preferences, and assessment of therapeutic response and side-effects for individuals participating in programs under the umbrella of Recovery & Reintegration.

- Prescribe medication based on symptoms and other information obtained from interviews as well as provide education on proper medication use to both client and/or client collaterals.
- Provide medical management of side effects (e.g., weight gain, blood pressure, etc.), including evaluation of client symptoms and side effects during each meeting.
- Assess client status through interview, observations, and assessments of problems and outcomes, including continually assessing for suicidality.
- Collaborate with client, treatment team, and other collateral contacts to develop and implement an individualized treatment plan.
- Provide health education to Adult Case Management and/or Substance Use clients and guidance to respective program staff.
- Coordinate health information exchange with RNs, MCOs, community medical providers, etc. in accordance with privacy laws.
- Respond to inquiries from external partners and communicate with external partners regarding refill requests.
- Remain familiar with emergency procedures.
- Ensure adherence to ethical standards of treatment, including NASW Code of Ethics.
- Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner.
- Medical Degree or Master of Science in Nursing required.
- Active Illinois License (APRN, PA-C, or MD) in good standing with practice authority to provide Medication Management services required.
- One (1) to three (3) years of experience in behavioral health, community health, and/or substance use field preferred.
- Demonstrated proficiency in EHR documentation and review required and the ability to complete all documentation in a timely manner and in compliance with the standards of LSSI and other regulatory bodies.
- Familiarity with accreditation and licensure standards preferred.
- Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
- Excellent organization, presentation, and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
- Annual TB test required.
- Background check clearance required.
- Valid driver’s license, in good standing for the state of residency required.
- Access to reliable transportation required.
- Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
