Are you a dedicated Advanced Practice Provider looking to make a difference in the lives of patients managing chronic kidney disease and endocrine disorders? We’re seeking a Nurse Practitioner or Physician Assistant to join a collaborative care team providing high-quality, patient-centered care in both inpatient and outpatient settings.

What You’ll Do:

• Work closely with nephrology and endocrinology physicians to manage patient care across hospital and clinic settings
• Conduct comprehensive health assessments, develop differential diagnoses, and implement treatment plans in alignment with state and federal guidelines
• Provide patient education and long-term management for chronic kidney disease patients
• Participate in dialysis rounding, chronic kidney disease clinics, hospital rounding, and on-call coverage as required

Schedule & Coverage:

• Typical hours: Monday–Friday
• Weekend rotation: Every 3rd weekend for hospital rounds
• Dialysis rounding: 1 shift, 3 times per month
• Daily mix: ~50% hospital rounding / 50% office visits
• Patient load: 15–20 patients per day
• Mileage reimbursement provided for hospital rounds longer than the clinic commute

• Washington Hospital
• Uniontown Hospital
• Mon Valley Hospital

Qualifications:

• Board Certified Physician Assistant or Nurse Practitioner
• Active Pennsylvania license
• Experience in nephrology, endocrinology, or internal medicine preferred
