Job Summary

Our client is seeking an experienced Nurse Practitioner (NP) specializing in neurology and chronic care management (PCM/CCM) to provide proactive, patient-centered care for individuals with neurodegenerative diseases such as Alzheimer’s, Parkinson’s, dementia, and mild cognitive impairment (MCI). This role is based in the San Francisco Bay Area with flexibility for hybrid or remote work, depending on patient and clinic needs.

Job Responsibilities: Deliver telehealth follow-up care for patients with chronic neurological conditions. Develop, document, and maintain care plans compliant with Medicare PCM/CCM requirements. Collaborate with neurologists and healthcare providers to ensure seamless care coordination. Lead and support Medical Assistants to ensure clinical excellence and teamwork. Communicate effectively with patients, families, and caregivers to promote treatment adherence. Record and submit accurate time-based activity logs for compliant CPT billing. Partner with the care operations and AI teams to enhance workflows and care processes.

Qualifications: Active, unrestricted Nurse Practitioner license in California (multi-state licensure a plus). Minimum of 2 years of experience in neurology, PCM/CCM, or chronic disease management. Familiarity with Medicare billing codes and documentation standards. Strong communication skills with empathy toward older adults and caregivers. Proficiency in digital health platforms and EMR systems. Reliable, organized, and proactive in coordinating patient care. Preferred: Experience managing neurodegenerative disease populations. Background in telehealth or hybrid-care settings. Multi-state NP licensure (CA + TX/AZ/FL/NY preferred). Leadership or training experience in PCM/CCM program operations.,
