Position Description
Penn Medicine at Home is looking for an Advanced Practice Provider (APP), to join our team. The APP serves as a professional clinical and administrative consultant supporting strategic initiatives within the Penn Medicine at Home system (including projects such as One Ambulatory Pharmacy and the Infectious Disease (ID) Shuttle). This role is pivotal in bridging clinical operations and system strategy, ensuring seamless integration and execution of innovative care delivery models.
The APP functions as a liaison to health system providers, offering expert-level support in both clinical and administrative domains. Responsibilities include electronic medical record (EMR) order entry, laboratory result review, and direct patient communication as appropriate to ensure continuity and quality of care. The APP collaborates closely with interdisciplinary teams to optimize workflows, enhance patient outcomes, and support strategic goals.
This position requires a strong understanding of healthcare regulations and standards. The APP is accountable for delivering services in compliance with local, state, and federal guidelines, and upholding Penn Medicine’s commitment to excellence, innovation, and patient-centered care.
Responsibilities
Nurse Practitioner candidates must be licensed to practice professional nursing in the Commonwealth of Pennsylvania and preferably licensed to practice in New Jersey. If the candidate is not licensed to practice in New Jersey, must be willing to obtain license. Candidates must also possess a MSN or DNP.

Responsibilities
- Develop, participate and implement multidisciplinary strategic plans to improve the program across UPHS.
- Serve as a liaison between clinical services, PMAH strategic program and patients.
- Collaborate with providers to create and implement appropriate therapeutic regimens.
- Provides direct clinical care for patient population according to established protocols and utilizes evidence-based practice standards in collaboration with team.
- Maintains accurate and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations consistent with organizational standards in the electronic health record (EHR).
- Identifies self properly to the patient and family and clarifies the role of the nurse practitioner if necessary.
- The nurse practitioner should function within their scope and refer to the collaborating physician when outside of scope, and/or directed by the collaborating physician or when requested by the patient and/or family. Performs interventions specific to assigned patient population and collaborative agreement.
- Participates in performance improvement processes to meet quality of care and service standards.
- Demonstrates awareness of professional responsibility as role model to peers and members of the health care team.
- Addresses patient and family concerns and creates a therapeutic relationship to provide support.
- Provide patient education, counseling and instruction regarding patient diagnosis, medical treatments, medications, and preventative care.
- Participate in and support accreditation, compliance and regulatory activities of the organization.
- Identifies patient/family needs for community resources and refers appropriately.
Nurse Practitioner candidates must be licensed to practice professional nursing in the Commonwealth of Pennsylvania and preferably licensed to practice in New Jersey. If the candidate is not licensed to practice in New Jersey, must be willing to obtain license. Candidates must also possess a MSN or DNP.
- PA and NJ CRNP licensure (CRNP) required. If candidate only has PA license, candidate will still be considered if they are willing to obtain their NJ license upon hire.
- PA and NJ Prescriptive Authority required. If candidate only has PA Prescriptive Authority, candidate will still be considered if they are willing to obtain their NJ Prescriptive Authority after obtaining their NJ licensure.
- PA and NJ RN licensure (RN) required. If candidate only has PA RN license, candidate will still be considered if they are willing to obtain their NJ RN license.
