Position Description

Position Summary

The Nurse Practitioner (NP) will provide flex coverage across Transplant (initially lung and heart) and Heart Center areas, with primary responsibilities in Advanced Cardiac Therapies (ACT) outpatient and inpatient care on Team 3 ACCU Medical, managing patients with cardiomyopathy, heart failure, and pre-/post-transplant needs.

This position offers flexible and adaptable coverage throughout Transplant and Heart Center programs, providing exposure to both pediatric and adult populations. The NP will collaborate with multidisciplinary teams to deliver high-quality, patient- and family-centered care.

Key Responsibilities

Heart and Lung Transplant

  • Provide comprehensive inpatient and outpatient care for heart and lung transplant patients across the full continuum—pre-transplant education, evaluation and management, transplant surgery, and long-term follow-up.
  • Train and collaborate with solid organ transplant NPs (heart, liver, kidney, intestine) to support cross-coverage and program integration.
  • Monitor and manage immunosuppressive therapy, instituting appropriate diagnostic and therapeutic plans for rejection and infection in collaboration with physicians.
  • Serve as a core member of the multidisciplinary transplant team, acting as an educational resource for staff, patients, and families.
  • Participate in program and protocol development, discharge planning, community education, quality improvement, database management, and regulatory compliance.

ACCU Medical Team

  • Provide care for pediatric and adult patients with cardiac medical conditions including congenital heart disease, arrhythmia, heart failure, and pre-/post-heart transplant management.
  • Manage non-cardiac medical issues for patients admitted to the Cardiology service.
  • Coordinate care for acute and chronic medical issues, ensuring seamless transitions and optimal patient outcomes.
  • Collaborate closely with multidisciplinary and consulting teams to deliver comprehensive and family-centered care.
  • Participate in complex discharge planning and continuity of care efforts.

Advanced Cardiac Therapies (ACT)

  • Partner with physicians, NPs, nurses, administrative, and multidisciplinary team members to deliver coordinated care across inpatient, cardiac cath suite, CICU, and cardiac clinic settings.
  • Maintain frequent contact with patients and families to support longitudinal care and home-based needs.
  • Participate in daily rounds, weekly team meetings, and clinic sessions, including documentation, billing, prescriptions, prior authorizations, referrals, and education.
  • Provide cross-coverage for lung transplant patients as needed.

Schedule: 40 hrs./wk., day shift with the possibility of nights, weekends, and some holidays as program needs require.

Education

Minimum Qualifications

  • Graduate degree from an accredited Nurse Practitioner program.

Experience

  • 2+ years of pediatric RN experience required (Critical care, cardiology, or transplant settings strongly preferred)
  • New graduate NPs with 5+ years inpatient RN experience (Critical care, cardiology, or transplant settings strongly preferred) will be considered.
  • Experience in pediatric complex care management and/or outpatient clinic settings preferred.
  • Must be highly organized, with strong written/verbal communication, EMR competency, and attention to detail.
  • Demonstrated ability to work independently, manage time effectively, and solve complex clinical issues collaboratively.

Licensure/Certifications:

  • Current MA NP license with PNP or FNP certification.
  • BLS and PALS required.
  • Willingness to pursue Transplant Coordinator certification.

Sign-on bonus: $5,000–$15,000. Higher bonus for candidates with significant pediatric or cardiac experience. (For candidates who have not worked at BCH in the past 2 years.)

The posted pay range is Boston Children’s reasonable and good-faith expectation for this pay at the time of posting.

Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
