Mana Medical Group is a Los Angeles–based Internal Medicine practice led by Dr. Joon Suh D.O., specializing in the care of seniors across skilled nursing and assisted living facilities. Our mission is to provide compassionate, responsive, and reliable on-site medical care that improves outcomes and quality of life for residents.

Position Overview

We are seeking a part-time Nurse Practitioner to join our team and provide ongoing care for patients at several skilled nursing facilities (SNFs) and one assisted living facility (ALF) located in the Santa Monica / Westside area.

This is a great opportunity for an early-career NP who wants to gain valuable post-acute care experience, develop clinical independence, and grow with a supportive and expanding medical group.

Responsibilities

• Conduct initial H&P (History & Physical) within 3 days of patient admission
• Perform follow-up visits twice per month per patient (minimum)
• Manage chronic and acute conditions in collaboration with facility staff and supervising physician
• Document encounters accurately and promptly in EMR (PointClickCare, NextGen, etc.)
• Communicate effectively with nursing staff, families, and the interdisciplinary care team
• Participate in quality-of-care and readmission reduction initiatives
  
  

Schedule & Volume

• Approximately 60–70 patients per month across assigned facilities
• Flexible scheduling with autonomy to manage your own rounds
• Part-time hours, ideal for balancing with another clinical role
  
  

Qualifications

• Active California Nurse Practitioner License and DEA registration
• Board Certified
• Strong clinical judgment and communication skills
• Experience in geriatrics, internal medicine, or SNF/ALF settings preferred (but not required)
• Reliable transportation to multiple facilities within the Santa Monica / Westside area
