A reputable healthcare organization is seeking an experienced Nurse Practitioner to provide primary and preventative care services to residents in skilled nursing facilities. This role is ideal for a clinician passionate about caring for adults and seniors with complex and chronic conditions in a long-term care environment.

Key Responsibilities

• Provide primary and preventative care to long-term care patients residing in skilled nursing facilities
• Manage residents experiencing acute changes in condition
• Conduct advanced care planning and chronic care management
• Prescribe appropriate medications, including controlled substances, in accordance with Pennsylvania law
• Order, review, and follow up on treatments, diagnostic tests, and lab results
• Communicate regularly with nursing home staff regarding changes in patient condition and care plans
• Collaborate closely with the primary care physician to ensure comprehensive, patient-centered care

Must-Have Qualifications (3–5)

• Current, unrestricted Nurse Practitioner license in the state of Pennsylvania
• National Board Certification
• Active DEA licensure
• Clinical background in adult, family, or geriatric specialties
• Experience working in a long-term care or skilled nursing facility setting
• Proficiency with EMR systems
• Ability to manage patients with advanced illness and multiple chronic conditions
