Position Description
Position Overview
As a Certified Wound Care Nurse Practitioner, you will provide comprehensive evaluation and management of patients with acute and chronic wounds and related skin disorders. Care is delivered primarily through in-person visits and supported by virtual services when appropriate. This role emphasizes clinical excellence, patient safety, and adherence to CMS guidelines to ensure the delivery of medically necessary, high-quality, individualized care.
Key Responsibilities:
- Conduct in-person evaluations for patients with acute and chronic wounds in homes, ALFs, and SNFs within an assigned geographic area.
- Perform debridement and other wound care procedures in accordance with CMS guidelines to promote healing and prevent complications.
- Provide telehealth evaluations and digital follow-ups to ensure continuity of care.
- Maintain strict adherence to infection prevention and equipment handling protocols.
- Document all encounters and billable services in the EMR clearly, accurately, and within one business day, consistent with CMS requirements.
- Use critical thinking and evidence-based assessment to identify patient needs and develop care plans.
- Collect comprehensive patient data (biological, physical, functional, psychosocial) to inform treatment decisions.
- Prescribe medications and treatments within scope of practice.
- Collaborate with both virtual and field-based clinicians to coordinate and delegate care effectively.
- Partner with other healthcare providers (e.g., home health, primary care) via EMR and digital platforms to ensure seamless care transitions.
- Participate in quality improvement initiatives, process evaluations, and program development efforts.
- Provide leadership and input on documentation standards, billing processes, and clinical quality measures.
- Engage in team meetings and ongoing training to support continuous program growth and excellence.
Qualifications:
Active Nurse Practitioner (NP) license in good standing.
WOCNCB certification OR Wound Care Certified, Certified Wound Specialist with a minimum of 3 years working as a wound specialist.
Minimum of 3 years’ experience in wound care, home health, or geriatric care preferred.
Strong knowledge of CMS documentation and billing standards.
Excellent clinical judgment, communication, and time management skills.
Comfortable using EMRs, telehealth platforms, and digital care coordination tool
