Position Description
Monterey County, California: A well-established, fast-growing post-acute care practice seeks a full-time Nurse Practitioner or Physician Assistant to join its clinical team. This unique inpatient role offers significant autonomy within a highly supportive model that includes a hands-on Medical Director and a dedicated Medical Coordinator who manages scheduling, communication, and workflow.
Your focus will be the meaningful, relationship-based care of patients in skilled nursing facilities that serve both short-term post-acute rehab patients and long-term residents. You will enjoy a stable weekday schedule, strong clinical backing, and the satisfaction of guiding patients through rehabilitation and long-term management with a manageable daily census.
This organization values excellent communication, sound clinical judgment, strong ethics, and a genuine desire to make an impact in a vulnerable population. Providers with backgrounds in primary care, internal medicine, hospital medicine, SNF care, geriatrics, or critical care will thrive here, though experienced NPs and PAs from other settings are also encouraged to apply.
Position Summary & Daily Scope
- Provide direct patient care for post-acute rehab patients and long-term care residents
- Conduct daily skilled visits, evaluate new admissions, coordinate rehab and discharge plans
- Monitor for acute changes; diagnose and manage common conditions
- Order and interpret labs, imaging, and other diagnostics
- Develop treatment strategies for chronic and infectious diseases
- Attend family meetings and provide patient/family education
- Collaborate with the Medical Director, rehab teams, nurses, and case management
- Maintain accurate records and participate in quality improvement initiatives
- Conduct scheduled long-term care visits every 30 days – more frequently as needed
Compensation & Benefits
- Very competitive salary: $160,000–$180,000 or more based on experience
