A leading healthcare provider in the Central New York area is currently seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join their team. The successful candidate will support the Independent Practitioner Panel (IPP) in issuing physician orders for individuals eligible for community-based long-term care services.

About the Opportunity:

• Schedule: Flexible appointment times
• Hours: Minimum 9 hours/week
• Setting: Community-based long-term care services

Responsibilities:

• Reviewing and recommending consumer assessments
• Maintaining knowledge of regulatory changes and clinical quality
• Validating assessment outcomes using clinical skills
• Collaborating on quality assurance and improvement initiatives
• Supporting program goals through education and communication
• Participating in internal meetings and performance reviews
• Performing other duties as assigned by management
• Conducting interview-style assessments (no physical/vitals required)

Qualifications:

• 2+ years as an Advanced Practice Provider (NP or PA)
• Master's Degree from an NP or PA Program
• Active New York State License
• Active Medicaid Number
• BLS Certification
• Experience with adult/geriatric populations
• Computer literacy and tech navigation
• Strong written/oral communication skills
• Interpersonal skills and adaptability

Desired Skills:

• Internal Medicine, Adult Health/Geriatrics, Family Practice, Community Health specialties
• Knowledge of long-term care programs
• Proficiency in languages such as Spanish, Russian, Haitian-Creole, Chinese, Korean, and/or Bengali
