Schedule: Flex M-F, 40 hours a week

Salary: $150-$175k

Job Description:

We are seeking a dedicated healthcare professional to provide exceptional care and support for patients with End-Stage Kidney Disease (ESKD). This role focuses on holistic patient management, education, and collaboration with multidisciplinary teams to ensure the highest quality of care.

Must Haves:

Current specialty certifications and license to practice as an Advanced Practitioner in the state Interested

Nurse Practitioner in the state of California 

Experience working in Nephrology, Internal Medicine, Cardiology, Primary Care, or Risk Adjustment. 

Comfortable traveling between facilities and working independently

Responsibilities are listed below:

Conduct routine and comprehensive ESKD rounds under the guidance of nephrologists.

Manage holistic patient care, including: 

Depression and mental health support

Hospitalization risk reduction

Transition of care and co-morbid condition oversight

Provide patient education on ESKD and kidney disease.

Perform annual disease management assessments.

Collaborate with nephrologists, dialysis center teams, and other healthcare providers.

Coordinate care with PCPs and specialists as clinically indicated.

Support discharge planning and continuity of care.

Must Haves: Current specialty certifications and license to practice as an Advanced Practitioner in the state Interested Nurse Practitioner in the state of California Experience working in Nephrology, Internal Medicine, Cardiology, Primary Care, or Risk Adjustment. Comfortable traveling between facilities and working independently
