Position Description

Job Summary

We are looking for a Nurse Practitioner to join Phoebe’s Rehabilitation Services Team. This team is dedicated to helping individuals overcome functional limitations brought on by injury, illness, or various health conditions. Our approach is holistic, and our goal is to assist you in achieving the highest level of functional independence for a safe transition back to your home environment.

Our Inpatient Rehabilitation Unit at Phoebe North Campus offers a tailored and comprehensive rehab program designed to address each patient's unique needs.

  • On-Site Medical Management: Our team includes dedicated rehab physicians who provide on-site medical management, ensuring a safe healing and recovery process.
  • Round-the-Clock Nursing Care: You’ll receive 24/7 rehabilitation nursing care, ensuring you have the support you need.
  • Intensive Therapy: Expect to participate in at least 3 hours of combined functional training and therapy for a minimum of 5 days per week. Our intensive approach accelerates your progress.
  • Holistic Approach: We believe in enhancing your overall well-being. In addition to therapy, you’ll engage in individual and group leisure and social activities, providing a balanced approach to your recovery.
  • Patient and Family Education: Knowledge is empowerment. You and your loved ones will receive education and training to ensure a successful recovery and transition back home.
  • Guided Discharge Process: Trained rehab professionals will assist you through the discharge process, ensuring a smooth transition from our care to your home.
  • Additional responsibilities include but not limited to: IPR rounding and face to face visits,; IPR admits and discharges; PM&R consults and follow-ups; assessing appropriateness for IPR or other post-acute discharge placements if not appropriate for IPR; contributing to through-put/continuum of care and LOS initiative

Qualifications

Master's Degree in Nursing Required

Work Experience

3 years experience in supervision and independent decision making in major nursing area preferred

Licenses and Certifications

Nurse Practitioners Certification (NP-C) required

BCLS - Basic Life Support required

Department Specific Certification preferred

Essential Functions

ASSESSMENT AND DIAGNOSIS:

Collects data in a systematic and ongoing process

Issues are identified and diagnoses are developed based on assessment data.

Prioritizes data collection based on the patient's condition and needs.

Identifies expected outcomes and plan based on diagnoses or issues.

Documentation

Documents and submits required information and data in a timely fashion.

Clearly and accurately documents designated processes, policies, products, service offerings, etc.

Ensures that documentation is tailored to expected readers / users.

Uses correct terminology.

Conforms to required style and format.

Outcomes And Evaluations

Develops expected outcomes that provide direction for continuity of care.

Involves patient/family and other disciplines to determine expected outcomes.

Modifies expected outcomes based on changes in the assessment of the patient.

Includes the patient and others involved in the care in the evaluation process.

Evaluates the plan in relation to patient responses and expected outcomes.

Documents the results of the evaluation.

Rn - Leadership

Directs the coordination of care across setting and among caregivers.

Engages in teamwork as a team player and a team builder.

Serves in key roles at work through shared decision making.

Defines a clear vision including goals and plans and measures progress.

Promotes nursing through participation in professional organizations.

Rn - Quality

Uses the results of quality improvement activities in nursing practice.

Uses creativity and innovation in nursing practice to improve care delivery.

Incorporates evidence based knowledge to initiate change in nursing practice.

Participates in quality improvement activities.

Additional Duties

Adheres to the hospital and departmental attendance and punctuality guidelines.

Performs all job responsibilities in alignment with the core values, mission and vision of the organization.

Performs other duties as required and completes all job functions as per departmental policies and procedures.

Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs).

Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.

Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs.

For non-clinical areas, has attended training and demonstrates usage of age- specific customer service skills.

Wears protective clothing and equipment as appropriate.
