Job Overview – Family Nurse Practitioner
The Family Nurse Practitioner provides high-quality, evidence-based primary care to patients across the lifespan in a fast-paced clinical environment. Working in close collaboration with a dedicated clinical care team, this role focuses on accurate assessment, diagnosis, treatment, patient education, and chronic condition management to support improved outcomes for an assigned patient panel. This is a safety-sensitive position requiring adherence to clinical, regulatory, and operational standards.
Key Job Duties
- Deliver comprehensive, evidence-based primary and family care, including assessment, diagnosis, treatment, management of health conditions, counseling, and disposition planning.
- Perform routine patient examinations and manage scheduled appointments to support positive health outcomes.
- Provide lifestyle and preventive health counseling; support patient engagement in wellness, behavioral health, preventive screenings, and chronic disease management.
- Complete diagnostic testing, interpret results, and coordinate referrals or additional care as needed.
- Collaborate daily with the clinical care team to support population health initiatives and improve overall patient outcomes.
- Practice autonomously while adhering to standards of care and state regulations.
- Communicate effectively with patients, caregivers, and colleagues using verbal, written, and electronic platforms.
