• Duration: 13-week contract
• Schedule: Monday – Friday, 8am – 5pm (occasional extended patient care)
• Shifts per Week: 5
• Shift Duration: 8-hour days
• Work Setting: 4 days in-person field visits, 1 day remote telehealth
• Patient Coverage: Manhattan borough only (Uber Business Account provided for clinical travel)
• Estimated Weekly Pay Range: $3750 - $4450
• Hours per Week: 40

Nurse Practitioner Qualifications – New York Healthcare Jobs

• Active Nurse Practitioner license in New York State
• Graduate of an accredited healthcare professions school; ANCC or AANP certification required
• Active DEA license and New York controlled substance registration
• Certification in Basic Life Support (BLS)
• Minimum 1 year of clinical experience with aging adults and complex care patients
• Experience in home healthcare settings (private homes, congregate living facilities) preferred
• Experience providing virtual care via telehealth preferred
• Managed care and/or value-based healthcare experience preferred
• Proficiency in healthcare software and computer skills required
• Bilingual Nurse Practitioners preferred
• Ability to travel within Manhattan (Uber provided for clinical travel)
• Note: PMHNP and NNP certifications are not eligible for this Nurse Practitioner job

Healthcare Responsibilities – Locum Tenens Nurse Practitioner

• Deliver compassionate, holistic patient care including assessments, urgent care, and chronic disease management
• Collaborate with multidisciplinary healthcare teams to support care transitions and address social determinants of health
• Provide high-quality clinical care through in-person home visits and telehealth
• Maintain accurate clinical documentation, medication reconciliation, and coding for patient encounters
• Participate in care team huddles, operational meetings, and professional development
• Engage with patients and caregivers to build relationships and improve quality of life
• Coordinate with external healthcare providers and clinical partners for continuity of care
• Participate in rotating on-call coverage (telephonic only, as needed)
• Perform other market-specific duties as assigned
