• Location: Cuyahoga County, Ohio 
• Position Type: Locum Tenens Nurse Practitioner, PRN
• Shifts per Week: 1
• Shift Duration: 8-hour field days; flexible start between 7:00 AM and 8:00 AM
• Contract Length: 26 weeks
• Estimated Pay: $105/assessment
• Travel: Daily travel to member homes (between 60-120 miles/day, territory-based)
• Coverage: County-wide, not facility-based
• Minimum Availability: 4 full days/month (weekday coverage preferred)

Nurse Practitioner Qualifications

• Active, unrestricted Nurse Practitioner license in Ohio
• Board Certification (ANCC, AANP, or AACN) required
• At least 1 year of clinical experience as a Nurse Practitioner (no new grads)
• Ability to work independently and travel solo to patient homes, including underserved areas
• Adaptability to urban, suburban, and rural routes within Cuyahoga County
• Strong organizational skills and flexibility to handle route changes, no-shows, and schedule adjustments
• Passion for geriatric patient care and a proactive, adventurous mindset
• Must supply own patient bag with basic medical tools
• Professional attire or dark-colored scrubs required (no lab coat needed)

Primary Responsibilities – In-Home Healthcare Jobs

• Conduct annual Health Risk Assessments in member homes (one-on-one visits, no follow-up care)
• Perform physical exams, medication reviews, and assess social/environmental factors
• Complete basic labs (A1C, fit tests; no blood draws required)
• Document all assessments using a Matrix-issued tablet with same-day submission
• Manage daily travel routes, adapting to changes and ensuring timely visits
• Collaborate with the Clinical Services team for support and quality assurance
• Maintain high quality scores and submit assessments for review
• Contact members prior to arrival, complete visits regardless, and leave flyers if members are unavailable
