Key Job Details – Maine Home Health Nurse Practitioner
- Employment Type: Locum tenens, PRN (part-time, contract healthcare job)
- Location: Cumberland County, ME and surrounding counties (travel radius up to 60 miles)
- Setting: Home Health – In-home patient visits
- Shifts per Week: 1 (minimum 4 full days/month, weekday coverage)
- Shift Duration: 8-hour field days (including travel)
- Estimated Daily Pay: $350 - $690 (per-assessment payment model)
- Start Date: December 15, 2025
- Contract Length: Ongoing PRN, minimum 6 month commitment
Nurse Practitioner & Physician Assistant Qualifications
- Active, unrestricted Nurse Practitioner license in Maine (ANCC, AANP, or AACN Board Certification)
- Minimum 1 year of NP experience (no new grads)
- Physician Assistant candidates accepted in Maine (1 year of PA experience and Board Certification required)
- Ability to travel independently to member homes in rural, urban, and suburban areas
- Adaptable to logistical challenges (route changes, no-shows, schedule adjustments)
- Strong commitment to patient care and a flexible, adventurous approach
- Must supply own patient bag with basic medical tools
- Professional attire or dark-colored scrubs (lab coat not required)
Home Health Nurse Practitioner Responsibilities
- Perform annual Health Risk Assessments for geriatric patients in their homes
- Conduct physical exams, medication reviews, and assess social/environmental factors
- Complete basic labs (A1C, fit tests – no blood draws)
- Document and submit assessments via Matrix-issued tablet (same-day submission)
- Manage daily travel routes, schedule changes, cancellations, and no-shows
- Maintain high quality scores and compliance with clinical standards
- Coordinate with Operations Manager and Clinical Services team for monthly schedules and field support
