Indiana Nurse Practitioner Job Details

• Location: Delaware County, IN
• Employment Type: Locum Tenens, PRN/Part-Time Nurse Practitioner
• Shifts per Week: 5 (minimum 4 full days/month; weekday coverage preferred, weekends a plus)
• Shift Duration: Part-Time, 8-hour field days (flexible start at 7:00 or 8:00 AM)
• Start Date: November 17, 2025 (flexible)
• Assignment Duration: Ongoing, monthly scheduling

Nurse Practitioner Qualifications

• Active, unrestricted Indiana Nurse Practitioner license (ANCC, AANP, or AACN Board Certification required)
• Accepted NP specialties: Adult Geriatric Primary Care (AGNP), Family Nurse Practitioner (FNP), Emergency Nurse Practitioner (ENP), Acute Care Nurse Practitioner (ACNP)
• Minimum 1 year of independent NP experience (no new grads)
• Comfortable traveling solo to patient homes, including rural and underserved areas
• Adaptable to route changes, no-shows, and schedule variability
• Strong passion for patient care and a flexible, adventurous mindset
• Must provide own patient bag with basic medical tools
• Business professional attire or dark-colored scrubs required (no lab coats)

Nurse Practitioner Responsibilities

• Perform annual in-home Health Risk Assessments for geriatric patients
• Conduct physical exams, medication reviews, and assess social/environmental factors
• Perform basic labs (A1C, fit tests – no blood draws)
• Submit assessments via company-issued tablet (same-day submission required)
• Travel daily to patient homes within assigned Indiana counties
• Manage 2–3 visits/day during ramp-up, increasing to 4–6 visits/day
• Maintain high quality scores and adhere to documentation standards
• Communicate with the clinical services team for support and quality assurance
• Handle logistical unpredictability, including route changes and last-minute cancellations
