Key Job Details – Nurse Practitioner (Huron County, OH)
- Position Type: Locum tenens / PRN Nurse Practitioner
- Location: Huron County, Ohio (Norwalk, OH 44857, and nearby areas)
- Practice Setting: In-home healthcare visits (travel up to 120 miles/day required)
- Schedule: Minimum 4 full days/month (preferably Mon–Fri); part-time, flexible days; weekend availability a plus
- Shifts per week: 5 (part-time, PRN)
- Shift Duration: 8-hour field days (including travel time)
- Estimated Weekly Pay: $480 - $533 (per-assessment pay model)
- Travel Stipend: $60/day for travel expenses
- Start Date: December 1, 2025
- Assignment Length: 26 weeks, with potential for contract extension
Nurse Practitioner Qualifications
- Active, unrestricted Nurse Practitioner license in Ohio
- Board Certification (ANCC, AANP, or AACN) as AGNP, FNP, ENP, or ACNP
- Minimum 1 year of nurse practitioner experience in clinical or home health settings (new graduates not accepted)
- Experience with independent practice and willingness to travel solo to patient homes
- Ability to adapt to rural, urban, and suburban healthcare environments
- Strong commitment to patient care, communication, and clinical excellence
- High tolerance for logistical unpredictability (route changes, no-shows, etc.)
- Must provide own patient bag with basic medical tools
- Professional attire or dark-colored scrubs (no lab coat required)
Primary Responsibilities – In-Home Nurse Practitioner
- Conduct annual health risk assessments for geriatric patients in their homes (1:1 visits, no follow-up required)
- Perform comprehensive physical exams, medication reviews, and assess social/environmental health factors
- Complete basic labs (e.g., A1C, fit tests) as needed—no blood draws required
- Document and submit assessments via company-issued tablet (same-day submission required)
- Travel daily to member homes within a 60-mile radius (up to 120 miles/day)
- Manage a caseload of 2–3 visits/day during ramp-up, increasing to 4–6 visits/day
- Collaborate with the clinical services team for support and quality assurance
- Maintain high-quality standards and compliance with all healthcare protocols
