This office-based role is part of a popular service providing integrated, team-based care to a diverse patient population. This is a full-time role in a modern, team-based medical home offering strong onsite resources and excellent clinical support.

This progressive practice is part of the state’s largest FQHC and community health center system, employing nearly 1,000 people, including more than 200 clinical providers, across 17 practice sites and service locations. Position Highlights

• Provide comprehensive outpatient primary care and build your own all-ages patient panel
• Develop long-term relationships with patients and families in a community-based setting
• Practice in a modern, fully integrated medical home with strong onsite resources and coordinated team support
• Work alongside an experienced care team focused on continuity, access, and quality outcomes
• Full-time role: 40 hours/week (34 patient contact hours/week)
• Flexible schedule: choice of 4 or 5 days/week
• Family medicine call – your share will average just one week per year
