Advanced Practice Provider (APP) – In-Home Chronic Renal Care
Location: Richmond, VA
Schedule: Hybrid | Full-Time | Monday–Friday
 
About the Role
We’re seeking Advanced Practice Providers to join a patient-centered, home-based care team for patients with chronic kidney disease (CKD) through end-stage renal disease (ESRD). This innovative model combines in-home care, telehealth, medication management, and patient education to improve outcomes, reduce hospitalizations, and deliver high-quality, evidence-based care to medically complex patients.
 
Schedule & Structure

• Hybrid role: 4 days visiting patients in their homes, 1 day remote for charting, telehealth, and administrative work
• No weekends or on-call responsibilities
• Patients are scheduled within a clustered territory for efficient travel
• Travel covered with mileage reimbursement and all associated expenses paid

 
A Day in the Life

• Conduct comprehensive initial assessments, follow-ups, and hospital-to-home transitions
• Perform hands-on, head-to-toe assessments and obtain vitals
• Manage medications and provide disease process education for CKD/ESRD
• Collaborate with an RN, Social Worker, and supervising physician to coordinate care
• See approximately 4 patients per day
• Visit durations: 90 minutes for new patients, 30–60 minutes for follow-ups

Ideal Candidate

• Active and unrestricted NP or PA license in the state of employment
• 2+ years of direct patient care experience, ideally in renal, chronic disease, or home-based care
