Position Description

Monday–Friday No On-Call No Panel Management Comprehensive Benefits
Are you a Nurse Practitioner or Physician Assistant looking for a flexible, autonomous role focused on preventive care instead of high-volume clinic visits?
We are hiring Full-Time Nurse Practitioners and Physician Assistants to complete Comprehensive Health Assessments (CHA) and Health Risk Assessments (HRA) for Medicare and Medicaid members in their homes and residential settings.
This role focuses on preventive care, early detection, and closing care gaps — not managing ongoing treatment plans or maintaining a patient panel.

Compensation & Incentives

Base Salary: $95,500 – $158,400 (based on experience & market)
Monthly uncapped incentive plan
Year-end bonus opportunity (up to $25,000)
Average annual incentive payout: $25,500
Mileage reimbursement
Full medical, dental, vision, and life insurance
401(k) + PTO + paid holidays
CE reimbursement + paid education time

Schedule & Work-Life Balance

Full-time schedule typically Monday–Friday, 8:00 AM – 5:30 PM
Assessments conducted Sunday–Saturday (flexibility available)
No overnight call
Limited travel (local territory-based, up to 15% overnight as needed)

What You’ll Do
During a 60-minute in-home visit, you will:

Perform comprehensive adult and geriatric assessments
Review medical history and complete medication reconciliation
Identify chronic conditions and care gaps
Conduct preventive screenings and point-of-care testing (as applicable)
Provide patient education and wellness counseling
Document findings accurately in EMR
Collaborate with PCPs and care teams when needed

This role does NOT require managing ongoing primary care panels.

This position is ideal for Nurse Practitioners or Physician Assistants who:

Enjoy autonomy and independent field work
Value meaningful, one-on-one patient interaction
Prefer preventive care over clinic-based volume
Are comfortable working in home and SNF settings
Want strong earning potential with predictable scheduling

Qualifications

Master’s Degree or higher from an accredited NP/PA program
Active NP/PA license in the state of practice
Board Certification (AANP, ANCC, AACN, NCCPA)
BLS certification
1+ year NP/PA experience preferred (new grads welcome to apply)
Valid driver’s license & auto insurance
Comfortable with EMR documentation
