IHA Per Diem Nurse Practitioner / Physician Assistant
Reports To: Manager of Care Delivery
Travel Requirement: Must travel within assigned borough (The Bronx – Bronx County)
Position Summary
The IHA Per Diem Nurse Practitioner/Physician Assistant performs in‑person, no‑cost annual preventive health risk assessments for Medicare, Medicaid, and Affordable Care Act members. Assessments are conducted inside the member’s home, following all health plan expectations and quality standards. This role is ideal for clinicians who value autonomy, flexibility, and meaningful one‑on‑one patient interactions.

The IHA Per Diem Nurse Practitioner/Physician Assistant performs in‑person, no‑cost annual preventive health risk assessments for Medicare, Medicaid, and Affordable Care Act members. Assessments are conducted inside the member’s home, following all health plan expectations and quality standards. This role is ideal for clinicians who value autonomy, flexibility, and meaningful one‑on‑one patient interactions.
Schedule & Compensation
- Flexible daytime and evening scheduling
- 60–75 minutes per appointment
- Fee‑for‑service compensation (market rate per completed assessment)
- Mileage reimbursed at the current IRS rate after the first 30 miles/day
- Complete required self‑paced and instructor‑led training within designated timelines
- Provide a minimum of 30 appointment slots per month
- Perform high‑quality, comprehensive in‑home health risk assessments
- Submit accurate chart notes within 24 hours of each visit
- Respond to charting queries within 48 hours
- Maintain strong communication with clinical leadership
- Attend periodic training sessions and team meetings
- Enter member homes professionally and respectfully
- Safely lift up to 20 lbs and bend/kneel as needed during assessments
- Strong computer and EMR proficiency
- Understanding of CMS and ACA risk‑adjustment models
- Excellent documentation accuracy and thoroughness
- Clear verbal and written communication skills
- Strict adherence to HIPAA and confidentiality standards
- Ability to make sound clinical decisions within policy guidelines
- Adaptability to changing schedules, environments, and member needs
- Positive, collaborative approach with members and clinical teams
- Ability to proactively identify and resolve member or provider issues
- Master’s or Doctorate from an accredited NP program (FNP, AGNP, ANP)
- OR Master’s degree from an accredited PA program
- Home health experience within the last 5 years (Required)
- 1 year of in‑home assessment experience (Highly Preferred)
- Active RN and NP or PA licensure in the state of practice
- Board certification: ANCC, AANP, or NCCPA
- Active Medicaid ID (state‑specific)
- Active Medicare PTAN
- Current BLS certification
- Valid, unencumbered driver’s license
- Proof of adequate auto insurance for the state of practice
